With not much of server administration tasks required on your part, that high setup fee shouldn't be deterring. CI Host pre-installs all necessary software to run your server and will periodically update the server OS and apply critical security patches. That means you can relax and concentrate more on your business rather than managing the server.
Intel Celeron 2.66GHz costs $99/m only. Daily backups can be added for $50/m extra charge.
What's worrying is their pricing on upgrade options. The base server comes with 512MB RAM only, and a $25 monthly charge to add another 256MB is definitely expensive. And so is the case with additional hard drive and bandwidth.
They also run a "Crazy Deals" section, which is regularly updated and features some of the very tempting bargains on a dedicated server. But after having a look at their Add-On pricing, we'll be reluctant to place an order.
Fast Servers

Fast Servers has years of hosting experience and is a reliable provider of dedicated hosting solutions. Their powerful servers do bear a premium tag though, with entry level P4 costing $300 plus when configured with RAID1, Sonic Firewall, remote reboot and backup services etc. But they are ideal for hosting resellers with good amount of money in their pockets looking for a trustworthy long-term partner.
Most of the server admin tasks are left to Fast Servers by default. This includes OS & other software upgrades. You can also opt for a $100/, support level that will take even the slightest admin worry off your mind.
InterMedia
Setup Fee: $250
They offer fully managed windows 2003 based servers. At $300 per month you can have a Dell PwerEdge 750 server, powered by Intel P4 2.8GHz processor, 256MB RAM and two 40GB hard disks on a RAID1 configuration.
Here also the add-ons will cost a bomb. All server admin tasks are managed by InterMedia.
